Featured Form: Design O'Blog

Niki Brown is a designer and blogger that we’ve [mentioned once before](http://www.wufoo.com/2011/01/24/partygoers/) here on the Wufoo Blog. This time we’ll take a look at this nifty form of hers:

Design O'Blog

If you’ve ever visited [Niki’s blog](http://www.nikibrown.com/designoblog/) before, then you visit [this form](https://nebrow.wufoo.com/forms/apps-you-cant-live-without/) on Wufoo.com, you’ll have no doubt whatsoever that it’s hers. It features the exact same header graphic and similar wood texture. Not only does this look good, but forms that are integrated with the look of a site have higher conversion rates!

This form of course uses a [custom theme](https://help.wufoo.com/articles/en_US/kb/Theme-Designer/), but you might be surprised to know how super simple the theme really is. No custom CSS is used at all. The theme simply uses a custom logo and custom wallpaper, both of which are able to link to images hosted anywhere on the web. Those two things drastically change the look of the form to fit Niki’s style.

The idea behind this particular form is to gather information from people about “Apps you can’t live without” for a blog post. Kind of a one-off short-lifespan idea. If you had to code the form yourself from scratch, it probably wouldn’t be worth it. But that’s the core of (pardon the self back pat) why Wufoo is so cool. Particularly the [Bona Fide plan](http://www.wufoo.com/signup/) and up, where you can create an unlimited number of forms. Any ol’ idea you think of can be made into a form. Right beside your important business forms, you could throw together a form to send to your friends asking what movie they want to see tonight and forward it to a report of the results. We do it all the time =)

Seen or created a nice looking form lately? Let us know in the comments.
